📄 Job description

Sully.ai | Head of Engineering | Mountain View (HQ) + Remote | Full-time We build the AI operating system for healthcare. Our flagship product is an AI medical scribe that generates clinical documentation in real-time during patient encounters — used by thousands of physicians across 6 countries processing millions of encounters. What makes this interesting technically: • Multi-agent LLM orchestration for clinical note generation (think: specialized agents for HPI, assessment, plan, each with domain-specific reasoning) • Real-time speech-to-text → NLU → structured clinical output pipeline (sub-second latency requirements) • LiveKit-based voice infrastructure for our AI receptionist that handles patient calls end-to-end • FHIR/HL7 integrations with 30+ EHR systems (Epic, Cerner, athenahealth, etc.) • On-premise deployments in regulated environments (UAE, enterprise health systems) • We ship fast — multiple deploys per day, small teams with extreme ownership Stack: TypeScript/Node.js, Python, React/Next.js, LiveKit, PostgreSQL, BigQuery, GCP/Azure, Terraform. LLM layer uses multiple providers with custom orchestration. We're specifically hiring: Head of Engineering - We need a player-coach who's built and shipped product as a founder or early engineer, then scaled teams. Not a "process person" — someone who codes, reviews PRs, and sets the technical bar while growing the org. ~8+ years experience, startup-only background preferred. Comp: Competitive base + equity. We're $20M Series A last year, growing fast.
